  • Medical emergency data card—It’s important for doctors to know your medical information in an emergency, whether you’re at home or away from home. This emergency data card contains your medical information. You can update it once a year at no cost (if you need to update it more often, it’s $5 per card). To get your card:
    • Complete the medical data form enclosed in the Vitality Plus Gold membership kit.
    • Indicate the type of card(s) you prefer (wallet size or half-size)
    • Mail in your completed form
    • You’ll receive your emergency medical data card(s) in four-to-six weeks. Keep it with you at all times. Good idea: Carry your card with your driver’s license, where it will more likely be found.
